Medical form must be SIGNED AND STAMPED by the doctor before being uploaded to National Sports ID.  Failure to do so will result in rejection of medical form.  Medical forms must be uploaded before August 1, if they are not uploaded by first day of practice, your athlete will not be able to participate until the form is submitted.
